We recommend waiting 3-5 days for a call from 844-224-5264 to gain your appointment or to clear up any eligibility issues.
As soon as your initial paperwork is completed, your first appointment will be scheduled with a therapist to go through your information and background to clarify the information you have placed on the intake form as part of an initial biopsychosocial evaluation.
After this evaluation, you will be assigned to a regular therapist and have an evaluation with a psychiatrist or Nurse Practitioner.

Community Behavioral Health is a Joint Commission accredited mental health clinic that meets the standards and requirements of current best practices. At their first visit, patients will undergo a biopsychosocial evaluation from a therapist. They will then be scheduled for a psychiatric evaluation with a doctor who may request for lab work (if not done in the last 3 months) and possible additional testing which will determine the best course of treatment.

As a result of working for the highest standards of care, all patients who receive care at Community Behavioral Health receive weekly or monthly therapy from an in-house therapist who collaborates with the psychiatrist in order to safely provide medications. As a result, Community Behavioral Health is unable to offer medication-only services.

Outside medical records with assessments and recommendations made by previous physicians or other care providers will be requested in order to continue care effectively.

After a comprehensive exam and review of previous medical records, Community Behavioral Health's psychiatrists may continue previous treatments from outside providers or will make evidence-based treatment recommendations meeting current clinical practice guidelines.

Our expert team will discuss with you if you're eligible for our Psychiatric Rehabilitation Program (PRP), Intensive Outpatient Program (IOP), School-Based Mental Health (SBMH) or Behavioral Health Home (BHH); we do not guarantee that you will see all of the mentioned providers in the first and consecutive appointments.

Consecutive missed appointments without prior notice will eventually lead to patient discharge.

Inappropriate comments or behavior to any staff, threats, or destruction of property can lead to immediate discharge.

You may be asked for a urine and blood sample periodically and on the first day you attend the clinic, we reserve the right to offer a re-schedule if you're unable to be tested.

Please be aware if you are on benzodiazepines or other controlled substances that are clinically contraindicated, they will be tapered for your health and safety.
PREPARING FOR YOUR VISIT
First In Office Visit
For new patients, please make sure on your first visit that you bring the following:

Any payment needed for services (CBH does not accept checks as payment for the first visit)

Any documentation required by the clinic or doctor. (i.e. guardianship forms, intake documents, copies of tests, etc.)

Be prepared to sign release & consent form for the clinic (i.e. schools, primary care physicians, etc.)
